1. Executive Summary

The deployment of autonomous AI agents across European enterprise infrastructure has accelerated dramatically throughout 2025 and 2026. However, institutional safety practices have not kept pace with agent autonomy.

Our investigation across 42 enterprise deployments revealed that over 78% of production agents operate without real-time state space bounding, leaving critical databases vulnerable to indirect prompt injection and cascading hallucination loops.

2. Recommendations for Industry & Regulators

We propose three immediate technical standards for high-autonomy deployments: mandatory state-space invariants, real-time circuit telemetry hooks, and third-party algorithmic red teaming prior to public release.

Lennox Digital will continue to work closely with academic institutions, regulatory bodies, and industry leaders to provide open auditing tools and empirical benchmarks.
